What is needed from ALL MPC Students interested in Financial Aid?

Financial Aid Checklist
Students need to ensure that all required documents are submitted.
Activate your MPC Account
All enrolled MPC Students have student/email accounts and for Financial Aid purposes you will need to activate your account.
Free Application For Federal Student Aid
The FAFSA will need to be completed online if you are interested in Federal funding, such as Pell Grant, Cal Grant, and student loans.
Cal Grant
In addition to the FAFSA, you can apply for a Cal Grant by submitting a school certified Grade Point Average (GPA) to the California Student Aid Commission by the filing deadline. There are two deadlines: March 2-for all colleges and September 2-for community colleges. MPC Students with 24 transferable units need to submit the form to Admissions & Records. High school students will need this form completed by his/her high school.
Board of Governors Fee Waiver
This for is for California residents, as determined by our Admissions & Records Office. Students eligible for this aid will have their enrollment fees and a portion of their parking permit waived. You can also fill out the online form by visiting http://www.cccapply.org/BOG_Waiver.
Student Information Form
This is a required form for all MPC Students. It gives Financial Aid Officers additional information about yourself for the academic year.
SAP Policy & Student Auhorization Form
Students that receive Federal Student Aid (and Cal Grants) are required to adhere to the MPC Satisfactory Academic Progress (SAP) Policy. These are your responsibilities as an MPC Student on Financial Aid.

What else do I need?

To know what additional forms you may need to complete your file, you will need to know if you are a Dependent or Independent student.

If you answer "NO" to all of the following questions, you are considered Dependent. If you answer "YES" to any of the questions you are considered Independent, and may be asked to provide additional documentation.

Were you born before January 1, 1988?
Are you married?
Are you a Veteran of the U.S. Armed Forces or currently serving on active duty for the purposes other than training?
Do you have children or other dependents who receive more than half of their support from you?
Are both parents decesed, were in foster care or were a dependent or ward of the court?
Are you or were you an emancipated minor or ina legal guardianship as determined by a court?
On or after July 1, 2010, did your high school or school district homeless liaison determine you were an unaccompanied youth who was homeless?
On or after July 1, 2010, did the director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development determine you were an unaccompanied youth who was homeless?
On or after July 1, 2010, did the director of a runaway or homeless youth basic center or transitional living program determine that you were an unaccompanied youth who was homeless or were self-supporting and at the risk of being homeless?

Students Selected for Verification

Dependent Student

Dependent Verification Worksheet
Dependent students selected for verification by the Department of Education will complete this form and attach a copy of the prior year's federal tax returns with W-2s for the student and parents.
Parent Asset Form
To be filled out if your parent(s) have real estate, a business or farm investment.
Parent's Statment of Resources
Complete this form if you or your parent(s) have indicated on your Verification Worksheet or Student Aid Report that your parent(s) did not have an income for 2009.

Independent Student

Independent Verification Worksheet
Independent students selected for verification by the Department of Education will complete this form and attach a copy of the prior year's federal tax returns with W-2s for the student and/or spouse.
Student Asset Form
To be filled out if you (or spouse) have real estate, a business or farm investment.
Student's Statement of Resources
Complete this form if you have indicated on your Verification Worksheet or Student Aid Report that you did not have an income for 2009.

Additional Forms

Ability to Benefit Certification
Address Change
To be filled out by student if he/she will be changing their mailing address.
Agency Certification of Untaxed Income
Consortium Agreement
Complete form if you will be attending another college/university while attending MPC.
Dependency Status Documentation
If you indicate on your FAFSA that you are an orphan/ward of the court (or were a ward of the court until age 18), an emancipated minor, in a legal guardianship or homeless youth.
Professional Judgment
This form is required if you need to request a change in income status, dependency status, or selective service status.
Student Authorization for Designee
Due to FERPA, Student Financial Services cannot disclose information about your file to relatives. You can allow access to your financial aid file to a relative by submitting this form.
Student Authorization for Mail
Due to FERPA, Student Financial Services cannot disclose information about your file to a third party. You can allow information about your financial aid file to be mailed to a third party by submitting this form.
