Step 1.2010-2011 Scholarship Booklet - Review the Scholarship Booklet thoroughly, before completing the scholarship application and forms. You may wish to download, review, and complete the Master Scholarship Application before filling out the online version.
Step 2.Master Scholarship Application - After completing the Master Scholarship Application online, please remember to printout a copy of your online Master Scholarship Application, sign and date it, and submit the copy to Patricia Chapman in the Student Financial Services Office by 6:00 p.m. on Monday, March 15, 2010. (This form should be filled out with Internet Explorer for best results, in terms of printing out your online Master Scholarship Application.)
Step 3. - Recommendation Form for MPC Faculty & StaffScholarship Recommendation Form - This form can be used in lieu of a letter of recommendation. Please inform the individual, who is submitting a recommendation on your behalf of the following: Recommendation Forms must be completed online by MPC faculty and staff. Letters of Recommendation must be sent as e-mail attachments to Patricia Chapman at pchapman@mpc.edu. MPC faculty and staff must use their MPC e-mail account only.
Recommendation Form for Off-Campus SubmissionScholarship Recommendation Form - This form can be used in lieu of a letter of recommendation. Please download and give this form to the individual you wish to complete it. Because the authenticity of e-mails sent from off campus cannot be verified, recommendation letters or forms coming from off campus, must be sent directly to the Student Financial Services Office in a sealed envelope.
Step 4. Scholarship Process Information Sheets - After completing the Master Scholarship Application and other forms online, please review the 2010-2011 Scholarship Process Information Sheets, to make sure you have met all the requirements. Incomplete applications and forms will not be processed.
