Financial Aid

WHAT YOU WILL NEED:

  *  Select your major from the MPC Major Advising Sheet List and print form(s).  The form you select must be the major you have declared at the Admissions & Records Office.

  *   You will need to print an SFS Education Plan Form and MPC Genreal Education, CSU General Education, or IGETC General Education Pattern Form which need to be attached to your Major Form.  Any other forms pertaining to your major can be attached, such as an ASSIST (www.assist.org).

HOW TO COMPLETE:

  *   Review your Unofficial Transcripts & complete your advising forms.  If you need assistance contact your academic counselor.

       a.        Write the year and term on Column “C” for completed courses on your Major Unit Review and

              General Education Pattern Form. Specify the educational institution if course was not taken at MPC.

       b.       ‘IP’ means In Progress.  If you are currently taking a class  put in the term you are taking the course

WHAT TO TURN IN:

        *   Complete an SFS Education Plan listing the courses you will be taking each semester/term.           

        *  Sign and Date the SFS Education Plan form and submit to Student Financial Services for evaluation.
