Apply for EOPS

You can apply for EOPS at any time (no deadlines).

The EOPS application/intake form is required of all students in the program.  Students who are new to our program and students who are continuing in our program are required to fill out a new intake form every year.  You can print a copy of it and then bring it into the EOPS Office. 

Am I Eligible?

To help students succeed, EOPS provides academic support services that are specifically designed for each student’s special needs. Students that are accepted and participate in EOPS must meet eligibility criteria which certify them as low income and educationally disadvantaged. In order to be accepted and to participate in EOPS, students must meet the following eligibility criteria:

- Must be a California Resident
- Must be eligible for BOG A or Bog B *
- Must be enrolled or plan to be enrolled full-time (12 units or more) unless also a student in Supportive Services **
- Cannot have earned more than 70 degree applicable units*
- Must have at least one educational disadvantage*

* Brand new BOG C students are not eligible for EOPS due to income guidelines. Continuing EOPS students that have a BOG C and have completed intake each consecutive semester with an EOPS counselor can be served. Continuing BOG C students will no longer be eligible for EOPS if they exceed 70 degree-applicable units or six consecutive semesters.

Instructions

You will need Adobe Acrobat Reader (Adobe Reader) software to open the forms on this Website. If you do not have it, you may download it free from the Adobe Website (http://www.adobe.com/). Look for a button or link "Get Adobe Reader." You may also pick up copies of all forms at the EOPS/CARE Office (Humanities Building, Room HU-209). If your BOG Fee Waiver Application is complete, you can complete the EOPS application in the office immediately or return it later.

To submit a form to EOPS/CARE,
1. Click on a link below to download a form.
2. Select "Open" to open the form. (You will need Adobe Reader. See paragraph above.)
3. Print the form. (Pull down the "File" menu and select "Print.")

How to Apply

1. Complete the BOG Fee Waiver Application from the Student Financial Services Website, click "Apply Now," then click "Apply for BOG."

2. Complete the EOPS Application.

3. Complete the EOPS/CARE Responsibility Contract.

4. Make a copy of the forms to keep in your own file.

5. Bring or mail the completed forms to EOPS.
