Application must be completed online in one sitting. If you leave this page, the information you have entered will not be saved.

Applications will be accepted from September 10th  until September 28th at 3 pm , for the class entering Fall 2019. Please complete all required fields before submitting your application. Application online will turn off at 3pm on September 28th. Official transcripts must be received in the nursing office by the close of the application cycle by 3 pm September 28th, in order for your application to be considered complete. (If you submitted transcripts during a previous application cycle, you do not need to duplicate.) It is your responsibility to assure that we have received them: A satisfactory TEAS assessment score is required prior to admission. This assessment test will be offered at Maurine Church Coburn School of Nursing (MCCSN) to selected applicants after Phase 1 scoring.

You must read this entire Application Information Packet before you submit your application. (It's a pdf document and is not contained in the form below). Forms to be submitted for multi-criterion points are in this packet. Please refer to the scoring tool on p. 24-27 and submit the required documentation to the nursing office by September 28th.

If you would like to be considered for advanced placement, please submit:  1) a copy of your active current California LVN license  2) official transcripts from LVN program of study  3) documentation (from employer, on letterhead) of at least one year of work experience as an LVN  4) official transcripts for required coursework.  Lifespan Development (PSYC 25) and Pharmacology (NURS 100) are not required for advanced placement, but are required for first semester placement
